ETV Satellite Receiver Order Form Information

ETV Satellite Receiver Order Form

The ETV Satellite Receiver form is used to submit your organization’s receiver request. An invoice will be sent to the billing contact you indicate. Please make payment upon receipt of the invoice.

Form Instructions:

In order to narrow the search for your organization, you are required to select your site type. Your choices are:

  • Higher Ed
  • State Agency/Gov
  • K-12 School
  • Business
  • Medical
  • PSN
  • Other

Select your organization’s county, which is also required.

You can type in the name of your organization but that is optional.

Organizations who meet the criteria you chose are listed. Find your organization and click on SELECT.

If you cannot find your organization please email your organization’s name, billing address, billing contact, phone number, program scheduling contact and email address to sokonek@scetv.org.

Check the current information we have and update as needed, including contact information.

The pricing for the receivers has changed. If you would like ETV to install the receiver the total cost is $200 each. If you choose to install the receiver yourself the total cost is $100 each. Regardless, all receivers purchased through ETV come preprogrammed.

Purchase order numbers are not required but preferred if available.

The amount billed is calculated for you after you enter the number of receivers and whether or not installation is requested.

Submit the order. Print the confirmation page. ETV Accounting will send you an invoice. Please pay upon receipt of the invoice. If you require the invoice by a specific date, please indicate that in the comments section.
